Usiak, Henry M.
Physician. Born in Buffalo, N.Y. Education: St. Stanislaus parochial school, Canisius High School, Buffalo, N.Y. Graduate of University of Buffalo, School of Medicine, Buffalo, N.Y., from which he received M.D. degree in 1941. Enlisted in the Medical Corps Reserve of U.S. Army in 1941. Called to active service in 1942 and commissioned as a first class lieutenant. Stationed in the beginning at Camp Carlisle, Pa. At present stationed at Camp Edwards, Mass., and assigned as captain of 54th Battalion U.S. Army Medical Corps. Residence: 622 Fillmore Ave., Buffalo, N.Y.
